1/29/2020:

Jolly went to BIR San Juan to submit documentary requirements for the new business registration of Beauwarts+. COR, "Ask for Receipt", and Authority to Print were released on this same day.

12/17/2019:

I went to BIR San Juan to submit documentary requirements for the new business registration of Beauwarts+. However, BIR did not accept the documents.

Issues:

1. The business address per SEC registration is at Lower Level Theater Mall, L64 Connecticut Lane, Greenhills, San Juan...while the business address per Mayor's permit indicates only units L61/62 Lower Level, Theater Mall. BIR is asking for the permit for unit L64.

2. BIR also requires the application for authority to print for Sales Invoice (as Retailer-skin products)

Requirements for BIR Registration:

1.       BIR Form No. 1903

2.       SEC Certificate

3.       Articles of Incorporation

4.       Mayor’s Business Permit

5.       Certificate of Lease issued by Greenhills Theater Mall

6.       Board Resolution appointing the authorized representative

7.       Copy of IDs of all the directors and the authorized representative

8.       BIR From No. 1906

9.       Final & clear sample of Official Receipt

Schedule of BIR payments:

1.       Annual Registration Fee with penalty…..P1,628.45

2.       DST on subscribed capital stock w/ penalty…..P2,621.04

3.       DST on lease contract w/ penalty…..P1,306.48

4.       Penalty for late registration……P1,000.00

Total Amount Paid:         P6,555.97

10/30/2019:

Received P5k from Beauwarts+ as payment for additional expenses for BIR registration.

10/23/2019:

Received P8k from Dan Caingcoy (Beauwarts+) as mobilization fund for processing of BIR registration of Beauwarts+.

Total estimated expenses for registration is P8,000.00 including registration fee, doc stamps tax on capital stock and lease contract, books of accounts and printing of official receipts.
