i.         Board Resolution appointing the authorized representative

ii.       Photocopy of IDs of the President and Secretary of the corporation and the authorized representative

iii.      Photocopy of COR

iv.     Last issued ATP is not applicable because the taxpayer has just transferred from RDO-Paranaque to RDO-West Cavite (the OR serial numbers will start again from 0001)

3:30PM – release of Authority to Print (ATAP)

Then, the original copy of ATAP was given to the printer; paid P1,500 for printing of 10 bklts of Official Receipt (to be delivered in 2 to 3-week time)

4:30PM – get “Ask for Receipt” poster; registration and stamping of the ff. books of accounts:

Paid BIR Annual Registration Fee with penalty for late payment.

Received P3k from J. Won as service fee for the processing of clearance from BPLO-Paranaque City re: Transfer of place of business from Paranaque to Gen. Trias, Cavite City. Reason for transfer: property was sold.

Status: Pending because BPLO requires that the new owner must secure its own business permit.
