Re: Cancellation of BIR registration; reason is that the application for mayor's permit was disapproved by BPLO.

Submitted the ff. documents for cancellation:
1. BIR Form 1905
2. Request for cancellation with request for waiver of penalties
3. RCS open cases verification
4. Original COR & "Ask for Receipt"
5. SPA of authorized representative and ID of owner.

To be assigned to a revenue examiner; call #8829-2135 to follow-up

Received P2k as service fee.

3:00PM – submitted the ff. documents for new business registration:

1.       BIR Form No. 1901

2.       DTI Certificate

3.       Copy of Mayor’s Permit, not yet available (told the revenue officer that it is under process)

4.       SPA appointing the authorized representative (paid P200 for notarial fee)

5.       Copy of ID of the taxpayer and authorized representative

6.       Copy of Lease Contract

7.       Copy of payment of BIR Registration Fee and DST on contract of lease
